This form is for the Williamsville studio for the 2011-2012 dance season. There is a separate form for the summer classes. 

The registration fee and first month's tuition are due at the time of registration, but no later than the first class. Each student must also return a signed tuition/fees policy. This policy sheet is given so all of our policies are clearly outlined and understood by all.  Also, each student must return a liability waiver.
